CCP Forms

Each CCP student is required to turn in the following forms:

When applying for the CCP program:

• CCP Application
• High School Transcripts – Submitted by high school
• Parent Signature Form – This is required for all non-public high school students
• CCP Permission Slip – Required for ALL students – uploaded HERE

Each academic year AND if anything changes in a student’s record:

• Updated High School Transcripts – Email CCP@lqqqhuanbao.com

For graduating CCP seniors:
